ESOZ Automatically Transfers Medical Data to a New Family Doctor

The Electronic Health System records every stage of a patient’s treatment. All medical data are stored in the central ESOZ database. The information remains accessible after changing a family doctor or transferring to another hospital. The digital medical card fully replaces paper equivalents and prevents the loss of important information.

The system automatically collects all key health indicators. This includes personal data, consultation details, diagnoses, treatment plans, and examination results. Electronic prescriptions, referrals, medical reports, hospitalization data, surgeries, and vaccinations are also recorded in the digital medical card. Healthcare professionals enter this information through the medical information system during each visit. Every entry is verified with a qualified electronic signature from the doctor. This ensures data authenticity and secure storage at the central level.

The medical history is preserved regardless of the choice of a specific specialist or medical institution. A new family doctor gains full access to previous records immediately after signing a declaration with the patient. Restrictions apply only to sensitive information regarding mental health or HIV status. Access to such data is granted only to the author of the relevant record. Other specialists can view these data only after obtaining direct patient consent.

Specialists see the medical history in the scope necessary for providing assistance. If needed, they request extended access to other medical records. The electronic system creates a secure environment for information storage. The patient freely chooses medical facilities, and their digital card synchronizes and travels with them.
