How to sign or change a declaration with a doctor — the main things patients need to know

A declaration between a patient and a doctor is a key document that confirms a person’s right to independently choose their primary care physician. After signing the declaration, the state, through the National Health Service of Ukraine (NHSU), pays for the services of this doctor within the framework of the Medical Guarantees Programme.

The doctor with whom the declaration is signed accompanies the patient in matters of prevention, treatment and monitoring of health, prescribes the necessary examinations, carries out vaccinations and provides emergency care. The declaration enables patients to receive these services free of charge within the state-guaranteed package.

Ukrainians can now sign a declaration with a family doctor, therapist or paediatrician. Each doctor has a specific optimal number of patients: up to 1,800 for a family doctor, up to 2,000 for a therapist and up to 900 for a paediatrician. If a doctor reaches this limit, they can decide independently whether to accept new declarations.

You can choose a doctor at any medical facility that has signed a contract with the National Health Service of Ukraine (NSZU). You can find out which doctors are accepting new declarations by calling the NSZU contact centre at 16-77, on the medical facility’s website, or on the online dashboard ‘Electronic map of primary healthcare service locations’.

Patients have the right to change their doctor at any time by simply signing a new declaration. If a doctor stops working or terminates the declaration with a patient, the institution is obliged to notify them. In such cases, patients can re-submit their declaration to another doctor.

If the declaration was made without the patient’s knowledge, this is a violation. The person can cancel it by submitting a new declaration or contacting the National Health Service of Ukraine via the website or contact centre.

Soon, it will be possible to submit or cancel a declaration online through the patient’s personal account, which is currently being developed. This service will simplify the process of choosing a doctor and make communication with the healthcare system even more convenient.

A declaration with a doctor is not a bureaucratic formality, but a tool that guarantees the patient’s right to quality medical care and clear interaction with the state in the field of healthcare.
