How to restore a veteran’s certificate if the document has been lost or damaged

In Ukraine, veterans can use not only paper certificates but also electronic certificates in the Diya app — both have equal legal force. The electronic document confirms the status of a combatant, a person with a war-related disability, or a family member of a deceased or injured participant in the Revolution of Dignity.

If the paper certificate is lost or becomes unusable, it can be quickly restored through the Ministry of Veterans Affairs, its structural units, or the Administrative Services Centre. The application is submitted in person, through an authorised person, or by post.

To obtain a new certificate, you must submit an application in any form, indicating your personal details, contact information and the method of receiving the document. The application must be accompanied by copies of your passport, identification code, a 3×4 photograph, and an extract from the Unified State Register of War Veterans, which can be obtained from the Administrative Services Centre. If the application is submitted by a representative, documents confirming their authority are required.

Servicemen who are serving or have served in the military can apply for the renewal of their certificate through the relevant security and defence authorities. Other categories — veterans, family members of those killed in action, persons with disabilities as a result of war — submit documents to the veteran policy departments at their place of residence or actual location, including internally displaced persons.

If you have lost your paper certificate, the electronic version in Diia remains valid. It entitles you to use all benefits and services without restriction while the process of restoring the paper document is ongoing.
